Membership Pricing

Membership fees include periodic provider oversight (typically every 3 months or as clinically indicated); access to hormone optimization, weight loss, peptide therapies, longevity care, and wellness services at preferred pricing, 10% off Thrive IV infusions, 15% off Designs for Health supplements, 10% off in-office supplements, optional monthly InBody scan for body composition tracking, one acute care visit per year, and one complimentary Myers IV infusion at one year membership anniversary. All services are subject to provider evaluation, medical necessity, and clinical discretion.

Membership fees are billed monthly via automatic payment with credit card on file. All payments are non-refundable. Failure to maintain a valid payment method may result in suspension or termination of membership. CareCredit may be used for non-membership services only. Member agrees to a minimum 3-month commitment. After the initial 3 months, membership may be canceled with 30 days written notice. Early cancellation prior to completion of the 3-month commitment will require payment of the remaining balance of the commitment term. No refunds will be issued for any payments made. If a patient elects to cancel or discontinue membership, they are no longer considered an active patient of Company. Re-enrollment at a later date will require a new medical consultation and payment of the current consultation fee. Company reserves the right to refuse, suspend, or terminate services or membership at any time for reasons including, but not limited to, safety concerns, inappropriate behavior, non-compliance with medical recommendations, or violation of clinic policies. No refunds will be issued for any unused portion of the membership term following termination. Member may cancel this agreement without penalty before midnight of the third business day after signing. If canceled within this period, a refund will be issued minus the value of any services already provided. You may request membership agreement for complete terms and conditions.
